How to Register
To register for this course, or to inquire for more details, please fill out our
on-line registration form.
Dates for this program are rolling throughout the year and are scheduled on a "first come, first serve" basis. You pick the date!
During the online registration process, you will be asked to provide us with up to three preferences for the dates you would
like to begin your course. We will do our best to accommodate your first choice.
The course is 4-days long, beginning on a Tuesday and concluding the following Friday. Each week of the year is available for a
maximum of two attendees per week. Attendees can be members from either the same or different practices.
Upon receipt of your registration information, our Scheduling Coordinator will review your requested dates and contact
you as soon as possible. Again, we will make every effort to accommodate your first choice. Once a date has been scheduled and
confirmed, our Scheduling Coordinator will mail you a packet with the specific details about your course.
Cost & Expenses
This course is free to attend. However, any related travel expenses, such as airfare, lodging and meals, are not covered.
Since this is a CME accredited program, we are hopeful that reimbursement for these expenses may be possible through your
practice. If you wish to receive Continuing Medical Education (CME) credit, there is a $40 application processing fee due at
the conclusion of the course, paid to the UW Continuing Medical Education Office.
