Evidence of GI bleeding
Anemia
Atypical chest pain

Heartburn present for > 5 years in person over age 40
Refractory heartburn despite medical therapy
Atypical symptoms
Evaluation for anti-reflux surgery*
Abnormal liver enzymes or jaundice

Otolaryngologist Persistent hoarseness
Cardiologist Persistent chest pain
Surgeon* Evaluation for anti-reflux surgery (in conjunction with a gastroenterologist)
